Senior portrait questionnaire

Updated October 17, 2026

Learn who the senior is, where to shoot, what to wear and what the school needs before the session.

Senior portrait questionnaire

Congratulations on your senior year. I want your photos to look like you, so these questions help me plan a session around what you love. There are no wrong answers, and you can skip anything you would rather not answer.

About you

What is your name, what school do you attend and what year do you graduate?

How would you describe yourself in three words?

Your interests

What are your sports, clubs, music and hobbies?

Is there anything you would like to bring to the session?

Instruments, a jersey, cleats, art, a letterman jacket, a car, a camera.

Location and style

Do you have a location in mind?

A park, your school, downtown, a field, the beach, a favorite spot in town. I can suggest places if you are not sure.

Which style are you drawn to?

  • Classic and polished
  • Casual and relaxed
  • Editorial or fashion
  • Urban and edgy
  • Natural and outdoorsy

How many outfits would you like to bring?

Two or three is typical. Each change takes 10 to 15 minutes.

  • 1
  • 2
  • 3
  • 4 or more

Comfort

Which features of yours do you like best?

Eyes, smile, hair, freckles, your laugh.

Is there anything you feel self-conscious about in photos?

A side of your face, your smile, a certain angle, glasses or braces. I will plan around it. You do not have to share if you would rather not.

Is there anyone, or any pet, you would like in some of the photos?

Yearbook and photo use

Does your school need a yearbook photo? What is the deadline and what are the requirements?

Dress code, background, pose, crop, file size. Please send me the instructions if you have them.

How do you plan to use your photos?

  • Graduation announcements
  • Wall art
  • Social media
  • College or scholarship applications
  • A mix of these

Parent contact

Parent or guardian name, phone number and email.

Who is paying for the session?

  • Parent or guardian
  • The senior
  • Someone else

How to use it

  • Address the form to the senior and copy the parent on the email.
  • Ask for the yearbook deadline and photo requirements early. Many schools want a specific crop or pose, and the deadline can come before the session date.
  • Use the style and self-conscious questions to choose locations and angles. A senior who feels understood is easier to photograph.
  • Confirm who is paying before the session and keep the parent's phone number handy on the day.

Questions

Who should fill out a senior portrait questionnaire, the senior or the parent?

The senior. They know what they want, and the session goes better when it is about them. Parents answer the contact and payment questions at the end.

Why ask about the yearbook before the session?

Many schools have a hard deadline and specific requirements for pose, crop and file size. If you ask early, you can book the session in time and deliver the right image.

How do I ask about insecurities without making a teenager uncomfortable?

Ask gently and make the question optional. Saying "you do not have to share" gives them an easy way out, and many will tell you anyway because they trust you.

How many outfits should a senior session include?

Two or three works well. Each change takes about 10 to 15 minutes, so more outfits mean a longer session or fewer locations.
